Primary Care Manager (PCM) Services

Reynolds Army Community Hospital has transitioned to Army Medical Home (AMH) teams by using the concept of Patient-Centered Medical Home (PCMH). PCMH is a new model of healthcare being adopted by leading healthcare systems across the country. The patient-centered medical home is a team-based model of care that provides comprehensive primary care for the entire family. In this model, a team of health professionals, coordinated by a physician, works collaboratively to provide high levels of care, access and communication with its patients. The physician-led team is responsible for providing all the patientís healthcare needs and, when needed, arranges for appropriate care with other qualified physicians. At its heart, the PCMH is healthcare the way it should be - easily accessible, patient-centered, healthcare provider team-based, and quality-focused. Appointments for all home teams can be made online at tricareonline.com or by calling 558-2000, Monday through Friday, 7:30 a.m. to 4:00 p.m.

The Army Medical Homes at Reynolds operate Monday through Friday, 7:30 a.m. to 4:30 p.m.

Family Medicine Teams include Service, Loyalty, Respect, Honor, Integrity, and Courage. These teams are all located on the first floor, north end of Reynolds and are designed for patients of all ages.

The Internal Medicine Team is designed for patients 18 and older. Internal Medicine also takes referrals from other physicians to evaluate patients and/or perform selected procedures such as EKGs, treadmills, and holter monitors. The Pediatrics is designed for patients from birth to 16 years of age. Pediatrics accepts referrals from other physicians to evaluate patients and/or perform selected procedures. The Fires Center includes 2 teams: Diamond and Leaders. Soldiers can access care by unit triage, calling the PCCC or booking an appointment online at tricareonline.com.
