The PHC is staffed by credentialed clinical pharmacists and a registered nurse. The PHC is designed to have a positive impact on patient outcomes by educating patients and providers, monitoring medications, and providing cost-effective therapy. Patients are referred by their PCM for education and/or management by the Clinical Pharmacist in the following areas: anticoagulation, asthma, diabetes, high cholesterol, and high blood pressure. Patients using 10 or more medications (prescription, over-the-counter, herbal or vitamins) can request a referral for a medication review.
