Other Primary Care Services


Reynolds has three separate clinics that provide specialized primary care services to TRICARE Prime and TRICARE Plus enrollees. They are listed below:



Immunization Clinic


Hours: 7:30 – 4:30 Monday – Friday

The Immunization Clinic staff is happy to provide our beneficiaries with all their immunization requirements. Whenever an immunization is given, the patient must be observed for 15 minutes before they are released. Therefore the last immunization will be administered at 4:15 p.m. Flu shots will be issued when the vaccine becomes available. Please bring patient’s yellow shot record and ID card. Allergy shots are administered only to beneficiaries whose PCM is at Reynolds. Walk-in services are on Monday mornings, Tuesday afternoons, all day on Wednesday, Thursday mornings, and Friday afternoons only. As with immunizations, the patient must be observed for 15 minutes after administration of an allergy shot. Schools age shots are typically given from the middle of June to the middle of August. The child’s shot records and/or any documentation of shots/immunizations is required. The child’s medical record is not required.



Exceptional Family Member Program

Overseas Screening


To be cleared for Overseas Travel, you will need the following:
1. Children: Current physical (within the last year) and shot records.
2. Women (spouse): Current well woman’s exam (within the last year) and shot records.
3. Men (spouse): Current physical (within the last year) and shot records.
4. Exceptional Family Deployment Screening Sheet, DA Form 5888, Soldier should complete #1-#7 and #8 is signed by the Soldier’s Personnel Section. Part B may only be signed by an authorized EFMP Physician.
5. Dependents outpatient records.
6. Children under 6 years of age need to be screened by EFMP Physician.
7. Call 580-558-2000 for an appointment for each family member traveling with sponsor.

Long Distance Overseas Screening


To be cleared for Long Distance Overseas Travel, you will need the following:
1. Children (spouse): Current physical (within the last year) and shot records.
2. Women (spouse): Current well woman’s exam (within the last year) and shot records.
3. Men (spouse): Current physical (within the last year) and shot records.
4. Exceptional Family Deployment Screening Sheet, DA Form 5888, Soldier should complete #1-#7 and #8 is signed by the Soldier’s Personnel Section. Part B may only be signed by an authorized EFMP Physician.
5. Screening of Family Members In Remote Oconus Areas, DA Form 5888-1, filled out and signed by treating physician.
6. Exceptional Family Deployment Program Screening Questionnaire, DA Form 7246, filled out and signed by treating physician.
7. Chronological Record of Medical Care, SF 600 from us, filled out and signed by treating physician (click here for forms).
8. Pediatric Development Questionnaire complete for each child under the age of 6 years.

The Exceptional Family Member Program (EFMP) is a mandatory enrollment program, established per Army Regulation 608-75, November 2006, that works with other military and civilian agencies to provide comprehensive and coordinated medical, educational, housing, community support and personnel services to Families with special needs. Soldiers enroll in the program when they have a Family member with a physical, emotional, developmental or intellectual disorder requiring specialized services so their needs can be considered in the military personnel assignment process.

All Family members must be screened and enrolled (if eligible) when the Soldier is on assignment instructions to an OCONUS area for which command sponsorship/ Family member travel is authorized, and the Soldier elects to serve the accompanied tour. This screening consists of medical records review for all Family members and developmental screening for all children 72 months of age and younger.

The Exceptional Family Member Program Office at Reynolds Army Community Hospital is located in Family Practice #1 Clinic, first floor. Come by to pickup a Summary Report or for information regarding enrollment criteria. The EFMP Office provides information and administrative assistance for family members enrolling in the Exceptional Family Member Program such as updating, enrolling and disenrolling.

Routine updates are scheduled with your Primary Care Manager or treating physician.





Population Health Clinic

Access: By appointment

The PHC is staffed by credentialed clinical pharmacists and a registered nurse. The PHC is designed to have a positive impact on patient outcomes by educating patients and providers, monitoring medications, and providing cost-effective therapy. Patients are referred by their PCM for education and/or management by the Clinical Pharmacist in the following areas: anticoagulation, asthma, diabetes, high cholesterol, and high blood pressure. Patients using 10 or more medications (prescription, over-the-counter, herbal or vitamins) can request a referral for a medication review.
